要旨

　未修復の修正大血管転位およびエプスタイン奇形を合併した妊婦に対する帝王切開術の周術期管理を経験した。妊娠後期に心不全を発症し，分娩後も肺うっ血が遷延した。体心室右室の機能低下を呈する妊婦の場合，妊娠後期から産褥期にかけての容量負荷に適応できない危険性がある。胎児の成長が良好であれば，満期産より早期での娩出により，心不全を回避できる可能性がある。先天性心疾患合併妊娠では，母体の非妊娠時および妊娠時の心機能と胎児発育の両者を考慮しながら，周産期管理を行う必要がある。

　We experienced the preoperative management for cesarean section in a parturient with congenitally corrected transposition of the great arteries and Ebstein’s anomaly. She developed congestive heart failure in late pregnancy, and after termination she suffered from pulmonary congestion for about two weeks. We should perform the perioperative management for a parturient with adult congenital heart disease, considering both the maternal systemic right ventricular function and the fetal growth.
