要旨

　肺水腫を合併した妊婦の緊急帝王切開において，高流量経鼻カニューレを用いて周術期呼吸管理を行った症例を経験した。術中は挿管を回避した呼吸管理を行うことができ，術後の集中治療室（ICU）での持続陽圧呼吸療法へつなぐことができた。導入は簡便であり，効果を理解して使用すれば手術室でも有用な呼吸補助デバイスであることが示唆された。

　A 32-year-old pregnant woman had been treated with ritodrine for a threatened premature birth. She had worsening pulmonary edema（a side effect of the ritodrine）, and she underwent an emergency cesarean section at 25 weeks and 2 days of gestation with the use of combined subarachnoid block and epidural analgesia with a high-flow nasal cannula. During the surgery, we were able to perform respiratory management without intubation and to seamlessly connect to continuous positive airway pressure therapy in the ICU.
