良性疾患に対する放射線治療の再評価と今後：変形性関節症を中心に

Re-evaluation of radiotherapy for benign diseases：a focus on osteoarthritis

我が国においては，放射線治療は悪性腫瘍の治療法の大きな柱の1つとして認識されている一方で，良性疾患についてはバセドウ眼症，ケロイドなどのわずかな疾患が適応となっているのみである。しかし，その歴史を振り返ると，X線の発見後まもない19世紀末から，様々な良性の炎症性・変性疾患の治療に応用されていた。例えば，1954（昭和29）年に出版された『疾患別放射線療法』（入江英雄著）をみると，三叉神経痛，変形性関節症など，良性疾患を含む様々な疾患に放射線治療が行われていたことがわかる^1）。20世紀半ば以降，薬物療法の発達や放射線被ばくへの懸念から，特に北米や日本ではその利用が大きく後退していた。

Low-dose radiotherapy（LDRT）, historically used for benign conditions, is being re-evaluated for osteoarthritis（OA）in aging societies. While LDRT is an established treatment for musculoskeletal pain in Germany, its use remains limited in countries like the U. S. and Japan. The therapy is thought to exert anti-inflammatory effects. Although numerous observational studies report high efficacy, recent randomized controlled trials have yielded inconsistent results, complicating the interpretation of its true efficacy. This article outlines the current evidence, safety profile, and future prospects of LDRT for OA.
