進行頭頸部癌に対する化学放射線療法（chemoradiotherapy：CRT）は手術に代わる有効な治療選択肢であるが，その治療効果や予後は限定的であり，また，患者のQOLを損なうような有害事象を引き起こす可能性もあり慎重な患者選択が必要とされる。¹⁸F-FDG PET/CT（PET/CT），MRI（拡散強調像），dynamic contrast enhanced MRI（DCE-MRI），perfusion CT（PCT）などの機能画像は化学放射線療法の治療効果予測のsurrogate markerとして期待されており，治療効果や予後予測に関する多くの検討が行われている^1）2）。さらに最近ではこれらの機能画像を組み合わせて腫瘍内組織環境を複合的に評価するmultimodality imagingによる検討も行われている^3）4）。

We designed the present prospective study to determine whether the combination of PET/CT, DWI, and perfusion CT can predict the treatment response to chemoradiotherapy in head and neck squamous cell carcinoma patients. ADC_mean for DWI and blood flow for perfusion CT were independent prognostic factors for predicting complete response after chemoradiotherapy, additionally, by combining ADC_mean with BF, the higher accuracy for the predicting complete response was shown compared with ADC_mean or BF alone.
