気管気管支骨軟骨形成症（tracheobronchopathia osteochondroplastica：TO）は気管および気管支の粘膜下に骨組織，軟骨組織が異所性に増生し，気管気管支内腔に突出する隆起性病変が認められるややまれな良性疾患である。画像検査では気管気管支の膜様部を除いた気管軟骨部に骨軟骨の増生を反映した結節病変が認められる特異的な所見が知られている。本稿では典型的な画像所見を呈し，病理学的所見も得られたTO症例について，文献的考察も加え報告する。

A man in his 70s had nonspecific cold symptoms found multiple nodules around trachea. The chest computed tomography revealed tracheal wall and left main bronchial wall were irregularly thickened with calcification. Bronchoscopic findings showed multiple whitish hard nodules located on the anterior and lateral wall of trachea and left main bronchus but posterior wall of trachea was free from these nodules. Biopsy and histopathology identified the submucosal tissue of bone and cartilage. Tracheobronchopathia osteochondroplastica（TO）is characterized by multiple submucosal osseous tracheobronchial nodules that spare the posterior wall. Because computed tomography allow an easy identification of TO, we should mention TO as one of the differentiation of tracheal diseases.
