肝胆道―びまん性肝疾患

Case presentation of diffuse liver disease with emphasize of the key radiological features

びまん性肝疾患の画像所見は，肝形態異常（肝腫大，変形など），肝実質濃度/信号異常，肝内血流/血管異常，肝内胆管異常，グリソン鞘領域異常（periportal collar/periportal abnormal intensity）などから主に構成される。これらの所見は，各びまん性肝疾患でオーバラップし非特異的であり，血液検査所見などの臨床情報を合わせて総合的に診断する必要があるが，各疾患に特徴的な画像所見も認識されている。また，近年のMRIによる定量的評価のアプローチも進んできており，びまん性肝疾患の診断において，MRエラストグラフィ（MRE），proton density fat fraction（PDFF），R2^＊mapなどの有用性が示されている。本稿では，総論として上記のびまん性肝疾患でみられる画像所見のうち主なものについて概説し，各論として主なびまん性肝疾患について実際の症例を提示しながらキーとなるCTやMRI画像所見などについて解説する。

Radiological features of diffuse liver disease mainly consist of hepatic morphological abnormality, density/signal alteration of the liver parenchyma, vascular/perfusion abnormality of the liver, intrahepatic bile duct abnormality, and the portal canal（Glisson’s sheath）abnormality. These features are non-specific and the diagnosis is usually based on combined the radiological features and clinical findings including biochemical examination results. In spite of the limitations, several specific radiological features of diffuse liver disease for the diagnosis are recognized. In this review, the major radiological features of diffuse liver disease are outlined and various representative cases are presented with emphasize of the key radiological features.
