日本における集中治療中に終末期を迎えた患者の家族ニードに関する文献研究

Family Needs of End-of-life Patient at Intensive Care Unit in Japan: A Literature Review

要旨

　集中治療から終末期へと移行した患者の家族ニードを明らかにすることを目的として研究を行った。国内文献データベースである医学中央雑誌（Web版Ver.5）を用いて過去28年間（1986年〜2014年）で検索を行い8件の文献を対象とした。急激な変化をたどった患者の家族は、「いつまでも生きていてほしい」と願い、「患者に触れ回復を願う」ことを希望していた。終末期へと変化したときには「何とか助けてほしい」という思いを抱きながらも「親しい人々に見守られ、穏やかに自然な死を迎えさせたい」、「患者を不憫に感じこれ以上傷つけたくないと思う」といった患者の安楽に関するニードも抱くようになり、生命への希望と安楽という2つのニードを抱いていた。

　I conducted a literature review to reveal the changes in needs of intensive care unit patients who shifted to end-of-life care from life-sustaining therapies. The material of this study was 8 literature published from 1986 to 2014 in Japan. The result indicated that the families of rapidly deteriorated patients wanted the patients be alive forever wishing the recovery by touching the patients. When the patients shifted to end-of-life care, the families hoped the patients would be saved at all cost, but at the same time, they wanted the patients peaceful death while surrounded by their relatives without experiencing any more hurt. The families have ambivalent needs of survival and comfort.
