◆要旨：脾捻転にて診断された遊走脾症例に対し，腹腔鏡補助下にポリグリコール酸（PGA）フェルトを細長いテープ状に加工し，脾下極の切痕部に巻きつけて左側腹壁にタッカーにて固定する新しい脾固定術を施行した．PGAは体内で吸収される人工素材で，生体材料由来の感染症のリスクがなく，強固な癒着を引き起こす性質を有し，若年発症の多い本疾患に適した素材であると考えられた．今回行った術式は現在広く行われている腹膜外パウチ法に比べて容易で確実な脾臓の固定が得られる有用な術式であり，今後遊走脾に対する腹腔鏡下手術に際しては考慮すべき術式の1つであると考えられた．

We report our experience with laparoscopic splenopexy for torsion of a wandering spleen using polyglycolic acid（PGA） felt. To treat this condition, we developed a novel technique that fixes the spleen by tucking the PGA felt around the inferior pole of the spleen to the left side of the abdominal wall. PGA is an artificial material that is absorbed in the human body, so there is no risk of infection from tissue-derived biomaterial. Furthermore, PGA causes firm adhesions that are favorable for fixation of the spleen. Since this procedure is safer and simpler than the widely used retroperitoneal pouch procedure, it should be considered as an option in case of laparoscopic splenopexy for a wandering spleen.
