双孔式回腸ストーマ脱出と傍ストーマヘルニアに対する腹腔鏡下修復術を行った1例

Prolapsed loop ileostomy with parastomal hernia repaired via laparoscopy : Report of a case

◆要旨：ストーマ脱出とはストーマの腸管が内腔に重積して腹腔外に脱出する病態であり，傍ストーマヘルニアに合併しやすい．今回筆者らは双孔式回腸ストーマ脱出と傍ストーマヘルニアを合併した症例に対し腹腔鏡下に修復術を行った．皮切前にストーマ脱出を用手還納した後，腹腔内を腹腔鏡下に観察すると肛門側腸管の重積は解除されていなかった．重積を解除すると，挙上腸管周囲に全周性のヘルニア門を認め，傍ストーマヘルニアと診断した．まずKeyhole法を用いて傍ストーマヘルニア修復術用メッシュを腹壁に固定した．さらに口側，肛門側双方の腸管を腹壁にそれぞれ4針で固定し，ストーマ脱出の予防を行った．双孔式回腸ストーマ脱出と傍ストーマヘルニアを合併した症例に対し，腹腔鏡下手術は病態を簡便に把握でき有用と考えられる．

Prolapsed ileostomy is common complication after stoma construction. It is often seen in conjunction with a parastomal hernia. We report a case of prolapsed loop ileostomy with parastomal hernia repaired via laparoscopy. Intraabdominal findings were ileal intussusception on the anal side and a giant parastomal hernia. The intussusception was released under laparoscopy. Precut Parietex parastomal mesh^Ⓡ was wrapped around the elevated ileum using the keyhole technique and was fixed over the hernia orifice. The ileum was sutured to the abdominal wall to avoid prolapse. The procedure is simple and useful for treating prolapsed loop ileostomy with a parastomal hernia.
