◆要旨：開腹手術では癒着防止吸収性バリア（以下，セプラフィルム®）が，術後の癒着防止を目的として広く用いられている．しかし，その物理的特性から，腹腔鏡下胆囊摘出術に用いるには工夫が必要で，過去の報告では準備の煩雑さ，特殊なデバイスや手技の習得の必要性など，汎用するには問題があると思われた．今回，手術用手袋をデバイスとし，簡便な準備と手技でセプラフィルム®を体腔内に迅速に挿入し，貼付ける方法を考案した．複数回の腹腔鏡下手術を経験する患者は増加しており，腹腔鏡下胆囊摘術における癒着防止を行う意義は高まってきている．セプラフィルム®を簡便に施行できる本手技は有用であると思われる．
A bioresorbale membrane （Seprafilm®） is used almost routinely in open abdominal surgery to prevent postoperative adhesions and intestinal obstruction. Yet, the application of Seprafilm in laparoscopic surgery （LS） is unfeasible because of its sticky properties. Previously reported methods did not result in widespread use because they needed expensive devices, techniques or time-consuming preparation before usage. Here we described an easy and prompt method of Seprafilm insertion and placement through 10mm laparoscopic ports using surgical gloves during laparoscopic cholecystectomy. Since the number of patients undergoing multiple laparoscopic surgeries is increasing, our method which prevents postoperative adhesion is beneficial for the majority of patients undergoing LS.
