要旨：近年，子宮内膜症患者の受診割合の増加に伴い，卵巣チョコレート囊胞を有する患者の割合も増加してきており，治療対象となる患者数も増加してきている．その中で低侵襲手術である腹腔鏡下手術は開腹手術に比べて圧倒的に有用であり，対象患者に対しては積極的に導入されるべきである．卵巣チョコレート囊胞に対する手術は，一見簡単にみえるが，子宮付属器周囲の癒着はほぼ必発であり，解剖や使用器具の特性，腹腔鏡下手術特有の鉗子操作方向の制限などについて十分な知識を得ていないと，手術操作が非常に危険になるということを熟知したうえで手術を行うべきである．
Recently, the necessity for laparoscopic cystectomy for endometrial cyst is increasing because of the increase in the number of endometriosis patients. The technique of laparoscopic cystectomy for endometrial cyst may seem easy and simple. However, it is very difficult because of the presence of severe adhesion around the adnexa and the limited movement of the instruments. The operator must have accurate knowledge and understanding of the nature of endometriosis and laparoscopic surgery.
