【コラム1】破傷風予防—ワクチンの追加接種で積極的な予防対策を

破傷風はワクチンにより予防可能な疾患である。先進国のなかでは，日本は高齢世代でワクチン未接種者の割合が高い。破傷風抗毒素抗体価は約10年で発症抑制レベルを下回るため，外傷患者で過去のワクチン接種から10年以上経過している場合には破傷風トキソイドを追加接種する必要がある。破傷風に対する予防投与のガイドラインは，成人期以降の追加ワクチン接種が進んだ米国のような先進国での推奨であり，小児期の定期接種以外の取り決めがない日本ではそのまま適用するのは問題がある。日本では1968年4月に破傷風ワクチンが定期接種となったため，1968年3月以前に生まれた外傷・熱傷患者では，破傷風トキソイドと抗破傷風ヒト免疫グロブリンの投与を積極的に考慮することが重要である。

Tetanus is a disease preventable via toxoid vaccination. Among developed countries, Japan has a high percentage of unvaccinated people in the older generation. Because tetanus antitoxin antibody titers fall below protective levels in approximately 10 years, additional tetanus toxoid should be administered to trauma patients if more than 10 years have passed since previous vaccination. The guideline for tetanus prophylaxis is a recommendation for developed countries with advanced vaccination systems, such as the United States, and its application to Japan is questionable. Since tetanus vaccine became a routine vaccination in Japan in April 1968, it is important to aggressively administer tetanus toxoid and tetanus immune globulin in trauma and burn patients born before April 1968.
