社会福祉を学び，専門とするソーシャルワーカーが医療の場にいる意味について考える。ソーシャルワーカーの専門職としての在り様を可視化し，言語化することが，多職種連携を実現させることにつながる。ソーシャルワーカーが加わる多職種連携では患者・家族の生活を知り，それに合わせた支援が展開できるように他職種と情報共有し，入院中にとどまらず生活をサポートする「支援」が退院後までシームレスなものとなるように，地域関係機関と医療の場における「支援」をつなぐべくソーシャルワークを活用する，その視点が不可欠となる。

Think about the meaning of being a social worker who studies and specializes in social welfare in the medical field. For social workers, visualizing and verbalizing the role of a professional will lead to the realization of multi-professional collaboration. Know the lives of patients and their families, and share information with other occupations so that support can be developed according to their lives. Social work should be utilized not only at medical institutions, but also so that the “support” that supports daily life will be seamless, and that it will be able to play a role in connecting “support” in the medical field with community-related institutions.
