慢性心不全の急性増悪により繰り返しICUに入室する患者や，強心薬やメカニカルサポートにもかかわらず回復しない症例など，終末期心不全は，急性期医療にかかわる医療者が日頃から葛藤を感じながら診療を続けている病態の代表例である。心不全の臨床経過を理解し，人生の最終段階のケアの提供を行うにあたり必要な概念を整理して理解することは，医療者の心理的負担を軽減し，かかわる人にとって納得感のある意思決定支援につながる。高度に進行した心不全患者を担当する集中治療医が知っておくべき慢性心不全の一般的な経過や予後予測の方法，心臓移植や補助人工心臓の概念や現状，人生の最終段階における意思決定の方法について解説する。

Patients repeatedly admitted to the ICU due to acute exacerbations of chronic heart failure and patients who do not recover despite inotropic drugs and mechanical support are typical examples of situations where medical professionals involved in acute care continue to feel conflicted daily. Understanding the clinical course of heart failure and organizing the concepts necessary to provide care at the end of life will reduce the healthcare providers' psychological burden and lead to adequate decision-making support for those involved.

　This paper will explain the general course of chronic heart failure and prognostic methods that intensivists caring for patients with advanced heart failure should know, the concepts and current status of heart transplantation and artificial hearts, and decision-making approaches in the final stages of life.
