Supplying adequate protein in critically ill patients with chronic kidney disease

慢性腎臓病（CKD）患者が重症化した場合のタンパク質制限の是非については，一定の見解は得られていない。しかし，CKD進展の機序であるhyperfiltration theoryに基づけば，タンパク質制限は理にかなっている。また，重症患者におけるタンパク質投与量の根拠となる臨床研究の多くは，腎障害患者が除外されている。現時点では，ICU入室初期はタンパク質制限をし，その後は窒素バランスなどを参考に調整していく，というのが現実的であろう。重症化したCKD患者のタンパク質制限の有効性を検討する質の高い無作為化比較試験が必要である。

Controversies remain regarding the pros and cons of protein restriction in critically ill patients with chronic kidney disease （CKD）. However, in accordance with the hyperfiltration theory which is the mechanism of CKD progression, protein restriction is reasonable. Many clinical studies which suggest no restriction of protein exclude patients with CKD. Currently, it is considered reasonable to restrict protein at the time of admission to the intensive care unit, and then to adjust the amount given based on nitrogen balance. High quality randomized controlled trials are needed to examine the effectiveness of protein restriction in critically ill patients with CKD.
