According to our previous investigation, patients suspected of drinking before visiting the ED (emergency department), were estimated around 1% of overall patients. Half of them were in the state of moderate or severe alcohol intoxication. Although a large part of such patients were delivered by ambulance service in the night time, they were not diagnosed as serious medical conditions, requiring hospitalization. Simple screening assessment revealed that half of them could be suffering from alcoholism. Alcohol related disorders are one of the major causes of consciousness disturbance at the ED. Direct measurement of blood alcohol concentration is very useful for differential diagnosis, but it is not always obtained at the ED in Japan. Osmolality gap is substituted for it, which shows an approximate value of blood ethanol concentration. Mean corpuscular volume and gamma-glutamyltransferase are commonly used biomarkers of heavy drinking. It can be said that ED has another important role to play for alcoholism, where the patient with alcohol-related illness should be screened, intervened briefly, and referred for special treatment.
