ヒトパピローマウイルス(HPV)感染は子宮頸癌発生に深く関与するが,HPV感染からみると,子宮頸癌に至るのはごく一部であり,むしろ例外的である.HPV感染は必要条件であり,感染をHPVワクチンで予防することで,子宮頸癌の発生を制圧することが期待できる.HPVワクチンが普及していくこの時期においても,HPV感染の自然史の解明が重要である.子宮頸癌発生の自然史としては数段階に分けて考えるのが妥当である.リスク因子がどの段階で作用しているかわかっていないものが多い.前方視的なコホート研究を行うことにより,リスク因子が作用する段階を捉えることができる.
Cervical cancer is the most important manifestation of genital human papillomavirus (HPV) infection. Persistent infection with carcinogenic HPV is the necessary cause of cervical cancer. However, a fraction of infected women develop cervical cancer. We all look with eager anticipation towards the prospects of HPV vaccines and the perspective of eradicating cervical cancer. Cervical cancer arises via several carcinogenic steps. In multistage cervical carcinogenesis, however, the step at which various risk factors exert their effect is not fully understood. We conduct prospective cohort studies to elucidate the step related to the risk factor.
