Risk management for the patient as a final goal of medical services

医学の急速な進歩と強力化は，医療における安全管理・危機管理・品質管理の導入を不可欠にした．各医療施設は危機(リスク)に関する管理体制および教育体制を構築して実施することが求められている．その中では医療事故(インシデント・オカレンス)を品質の“はずれ値”と捉え，経験深い熟練者(管理者)によって医療をよりよい業務手順のもとに整備し実施させる品質管理の向上が喫緊の課題である．

Sources of strong power such as nuclear energy should be properly controlled under safety, risk, & quality management systems.　In this day and age when medical technology has a powerful impact on the human body in ways good or bad, it should also be more precisely controlled than ever under these systems.　In the last decade, we aimed to create such management and educational systems at Tsukuba University Hospital.　Our final goal is the ongoing improvement of the overall quality of medical services provided under these systems, where all medical incidents, occurrences and accidents are considered as defects in quality, and every process of medical procedure is reviewed and revised by professionals with expert knowledge of such incidents (by the method we called the evidence-based PDCA cycle).
