A 30-year-old male presented with bilateral blurring of vision since one week before. His corrected visual acuity was 1.2 right and 1.0 left. Both eyes showed highly swollen optic disc, suggesting optic neuritis. Serological tests were highly positive for syphilis. Cerebrospinal fluid was positive for Treponema pallidum hemagglutination assay (TPHA) and showed pleocytosis. No intracranial abnormality was found by computerized tomography (CT) and magnetic resonance imaging (MRI). He was treated by systemic penicillin G at the daily dosis of 12 million units for 10 days with additional long-term systemic prednisolone. Serological and cerebrospinal fluid findings improved 15 weeks after initiation of treatment associated with disappearance of disc edema. These findings show that the optic neuritis occurred secondary to syphilitic meningitis.
