I．緒言

　上咽頭奇形腫は，上咽頭の正中部あるいは上咽頭側壁から生じるものがほとんどで，先天性奇形として出生時に発見されることが多い。成人になって初めて見つかる症例は稀であり，このような場合でも腫瘍は出生した時から存在していたが顕著な臨床症状を起こさなかったものと考えられ，後年腫瘍に随伴する何らかの症状とともに明らかにされる。本症例も出生時の詳細については不明だが，患者の話によれば生まれた時舌が2枚あるということで奇形腫を口の中に押し込め，その後50年近く顕著な臨床症状がなかったためにそのままになり，しかも腫瘍の先端が食道内に嵌入していた巨大な奇形腫のきわめて稀な症例である。

　Nasopharyngeal teratoma is rare and mostly found at birth or in neonates.

　A 52-year-old female presented severe discomfortable feeling of the throat ten days before visiting our clinic.

　Physical examination revealed a stick like tumor arising from the nasopharynx down to the left piriform sinus. Radiological examination showed that the tip of the tumor was extending into the esophagous.

　Under general anesthesia, the part of the tumor in the esophagous was extracted and its base was resected at the back of the soft palate.

　Microscopically, adipose tissue, sebaceous glands, hair follicles and squamous epithelium were identified, and those were consistent with dermoid tumor.
