I．はじめに

　アミノ配糖体系抗生物質であるネオマイシン（フラジオマイシン，以下NMと略す）は，現在注射剤はなく専ら点耳，点眼薬，貼付剤，含嗽剤等の局所治療薬として繁用されている。日常臨床上，あまりその副作用は問題とならないが，近年NM点耳薬による内耳障害の症例が報告されている。

　今回，全身熱傷治療にNM含有貼付剤（商品名：ソフラチュール）を使用し，漸次進行する感音性難聴をきたし，両側聾となった症例を経験したので報告する。

　A 55-year-old woman, suffered from burns on about 60%, of the body surface, was treated with topical application of Neomycin ointment (Sofratulle).

　The total amount of Neomycin was about 121g for a period of 17 months, and Gentamycin was about 5g for 12 months.

　Hearing impairment occurred in 11 months after treatment. Hearing loss in the higher frequency range made a progress along the entire frequencies, even after the therapy was discontinued. Vestibular signs were observed transiently for only several days in the 7th month of the treatment.

　Though the topical application of Neomycin is considered rather safe, its longterm and extensive use seems to increase the toxic effect as high as parenteral administration.
