鼻副鼻腔における真菌症の意義—特にAspergillus症を中心にして

FUNGUS INFECTION OF NASAL AND PARANASAL CAVITIES PARTICULARY FROM THE VIEWPOINT OF ASPERGILLOSIS

　I．はじめに

　最近における抗生物質，ステロイドホルモン，制癌剤などの薬物の頻用によつて真菌感染症は増加傾向にある。耳鼻咽喉科領域における真菌症は稀有なる疾患には属するが，その臨床所見よりみて診断上注目すべき諸点を有する。しかし最近われわれは上顎洞にAspergillus ballを形成するとともに骨破壊を伴つた症例を経験したので，その症例を中心として鼻副鼻腔の真菌症について検討を加え，かつ文献的考察を行なつた。

　Fungus infections involving the nasal and paranasal cavities appear to be relatively rare in occurrence, especially so with that of aspergillosis. However, with the advent of therapy with antibiotics, steroid hormone and anticancerous agents, the incidence of such infection seems to have increased.

　The authors report a case in point with a review of literature.

　A woman, aged 48 complained of a swelling of the right cheek. The x-ray examination showed a small dehiscence of the bony wall of the maxillary sinus. The patient was operated upon for opening the maxillary sinus which revealed a ball-like mass of tissue in this cavity. Pathologic examination of this mass proved the ball to be a growth of aspergillus fumigatus.

　This case is particulary interesting in that it presented symptoms resembling that of a malignant tumor by causing cheek swelling and evidence of bony destructions in the x-ray picture.
