耳鼻咽喉科におけるピリミジンペニシリン散（ピリペニ散）投与後の血中濃度と治療成績

　ペニシリンには各種の製剤があるが，近年，米国，英国，スウエーデン等においては経口投与法が行われ，30〜50％が錠剤として使用せられている。我国においてはペニシリンシヨツクが頻発する様になつて以来，筋肉注射等が敬遠せられる傾向にあり，シヨツク症状等の発生頻度の非常に少く，又起しても軽症ですむという経験にもとづいて，ペニシリンの内服療法に医師の関心が向けられ，漸次ペニシリンの経口投与法が広く行われ，満足すべき臨床効果をあげつゝある現況である。ペニシリンは酸に対して非常に不安定であつたが，1947年Behrens等^1）により発見生成されたPhenoxymethyl Pcはその後耐酸性であることがわかり，比較的高い血中濃度が得られることが報告されてから臨床的に用いられるようになつた。ペニシリンの経口投与に関してはJones及びFinland等^2）の報告が見られている。

　ピリペニ散の臨床成績については中川，中沢，藤井等各科領域における報告があるが，ピリペニ散による耳鼻咽喉科領域の治療成績の発表は少い。著者等は急性化膿性中耳炎，急性扁桃腺周囲炎，耳翼丹毒等についてピリペニ散の経口投与によつて得られた臨床成績並びに投与時間と血中濃度との関係について報告したいと思う。

Penicillin is manufactured in various forms. However, since cases of penicillin shock began to occur freguently in our country, the trend has been to avoid penicillin injections. Mo-reover, based on experience which indicates very few cases of shock symptoms and very mild symptoms when they do occur, a new interest has been shown among doctors in the internal use of penicillin, in which adequately successful clinical results have been obtained.

1) It has been discovered that the density of the medicine in the blood is conspicously greater when taken on an empty stomache as compared when taken after a meal. It is felt appropriate to administer 800,000 to to 1,200,000 units four times a day on an empty stomache.

2) Therapelltic results are as follows :

ⅰ) Forty-nine (73.1%) cases out of 67 cases of acute otitis media cured ;

ⅱ) Conspicious results recognized in four cases of tonsillitis and peri tonsillitis and two cases of erysipelas of the auricle.

3) No special symptoms were noted in the use of powdered penicillin.
