Ⅰ．はじめに

　トルコ鞍骨折は比較的稀な病態であるが，他部位の頭蓋骨骨折，脳挫傷，脳神経障害，髄液漏，尿崩症を含む下垂体機能低下症など，いくつかの合併症を来す例が多く臨床上その病態の認識は重要である^{1,3-6,8-14）}．今回われわれは，トルコ鞍骨折による髄液漏を呈した頭部外傷の稀な症例を経験し，その診断，治療にthree dimensional com-puted tomography（3D-CT）を含むCT所見，および術中内視鏡が有用であったので報告する．

Fracture of the sells turcica is rare and is associated with many complications. We successfully treated a cerebro-spinal fluid fistula caused by a fracture of the sella turcica. A 66-year-old male in a motor vehicle accident was admit-ted to an outside hospital with disturbance of consciousness. A computed tomography (CT) scan of the head revealed a subarachnoid hemorrhage and pneumocephalus. Cerebrospinal fluid rhinorrhea developed after admission. Repair of the fistula was attempted without success, and the patient was transferred to our hospital for further examination and treatment.
