Ⅰ．はじめに

　細菌性脳動脈瘤は稀な疾患であり，脳動脈瘤の1％以下とされているが^3），出血した場合は致死率が高く適切な治療法の選択が肝要である^1,2）．一般的に内科的治療に反応しないものに対しては外科的治療が考慮されるが，基礎疾患（感染性心内膜炎，弁膜症等）に基づく心機能の低下があり，また開心術と脳動脈瘤の治療のタイミングなど未解決な課題が存在する^6）．さらに，動脈瘤は末梢に存在することが多いため，開頭術，血管内治療のいずれにおいても，親動脈の温存の可否や，バイパスの必要など手技的にも困難な症例が少なくない^2,10）．

　われわれは1999年3月から2002年3月の3年間，治療対象となった脳動脈瘤588例のうち細菌性脳動脈瘤4例（4／588：0.7％）を経験した．これらの症例を呈示し，国立循環器病センターにおける細菌性脳動脈瘤に対する治療方針を若干の文献的考察を加えて報告する．

The surgical and/or endovascular treatment of four patients with infectious cerebral aneurysm associated with in-fective endocarditis was reported. Two patients presented with intracerebral hemorrhage. One of them, with large hematoma, was treated surgically and the other, with small hematoma, was treated by endovascular surgery. The third patient, with unruptured aneurysm of the distal anterior cerebral artery, was treated by endovascular parent-ar-tery occlusion without neurological deterioration.
