Ⅰ．目的

　頭部外傷におけるCT所見の1つとして，くも膜下出血が見られるという報告は多い．しかし，明らかな受傷機転があり，初回CTで外傷性くも膜下出血のみを認める症例について，その発生機序，打撲部位との関係，予後などにまで言及したものは少ない^29）．そこで，今回われわれは明らかな受傷機転があり，初回CTで外傷性くも膜下出血のみを認めた症例についてそれらのことに関して検討してみた．

Three hundred and fifty cases of patients with head injury were admitted to our hospital from 1998 to 2000. Among these cases, 10 cases manifested isolated, localized subarachnoid hemobrhage on the first computed tomo-graphic image after the trauma. Traumatic subarachnoid hemorrhage was found in the ambient, suprasellar, and syl-vian fissure. Bleeding was seen in the ambient cistern or suprasellar cistern at the site of the blow on the frontal or occipital area of the head. The bleeding was seen in the sylvian fissure at the site of the temporal head blow.
