初回手術より4カ月の経過で悪性転化した髄膜腫の1例

Early Recurrent Meningioma with Malignant Transformation:Case Report

Ⅰ．はじめに

　髄膜腫は，腫瘍の全摘出あるいはそれに近い摘出により，20年以上の長期生存が期待できる良性疾患ではあるが，Simpson grade Ⅰのような全摘出が行えたとしても，長期的には4〜15％の再発がある^2,3,10）．しかしながら，通常は良性の性格をそのまま維持するため，再手術により再び良好な予後が期待できる．その一方で，再発時の組織学的所見がたとえわずかでも悪化している症例は再発例の10〜38％と報告され^4,5），その中には悪性転化した症例も散見される．悪性転化した症例の多くは，再発を繰り返すうちに悪性転化しており，短期間で悪性転化した症例の報告はほとんどない．今回われわれは，初回手術で良性髄膜腫と診断され，全摘出を行ったにもかかわらず，4カ月という短期間で悪性転化した1症例を経験したので報告する．

It has been reported that the incidence of malignant transformation of meningioma is 10 to 38%. However, it is rare for a benign meningioma to recur with malignant transtormation shortly her surgery. We reported a case of re-current meningioma with malignant transformation 4 months after the initial surgery.

A 64-year-old female was adntitted for meningioma in the right parietal convexity on July 1, 1999. The tumor was totally resected on August 31st. There were no surgically nor histologically detected malignancies.
