頭部外傷後に出現した脳内気脳症の1例

Intracerebral Pneumocephalus Following a Head Injury:Case Report

Ⅰ．はじめに

　外傷性気脳症は骨と硬膜に亀裂部が存在し，その部を介して空気が頭蓋内へ侵入してくる病態であり，臨床的にしばしばみうけられる^8）．ところが，空気が脳実質内に限局する脳内気脳症は特異な条件がそろって初めて生じる病態と考えられており，その報告例も少ない．今回われわれは外傷後の脳内気脳症の1例を経験し，その発生過程をMRIにて詳細に検討できたので報告する．

We report a case of intracerebral pneumocephalus following a head injury. This condition is relatively rare, and only 14 cases, including the present case, have been reported to date. A 40-year-old man fell from a 3rd floor window on June 29, 1999. The patient was admitted to the hospital. Plain skull X-ray films revealed a left basal skull fracture, and CT scan revealed a small contusion at the left frontobasal lobe. The patient was treated conservatively.
