I．はじめに

　血管内視鏡の歴史は比較的古い^2,9,10）．しかし，用いる内視鏡が太すぎることや，血液を視野から排除することの困難さが問題となり，十分な時間良好な画像を得ることができず，開心術中など特別な場合を除けば臨床応用にはこれまで限界があった^{1,4,6,8,11,14）}．今回は0.6—0.75mmの直径を持つ微細径fiber catheter, double lu—men balloon catheterを利用し，頭蓋外閉塞性脳血管障害6症例に対して，血管内視鏡検査を行い有意な所見を得ることができたので，preliminary reportとして以下に報告する．

Flexible thin fiber catheter is applied as vascular en-doscope for occlusive lesions in the extracranial cere-bral arteries. This fiber catheter is introduced trans-femorally through 6 or 7 french-sized double lumen bal-loon catheters. Vascular endoscope clearly demon-strates the internal surface of these vessels by transient occlusion of the proximal side of the vessel through balloon inflation. The occlusive lesions in the subcla-vian artery or in the proximal vertebral artery were re-vealed as round smooth-surfaced mass to be treated by the percutaneous transluminal angioplasty.
