I．はじめに

　腰仙部脂肪腫やtight film terminaleなどの奇形によって引き起こされるtethered cord syndromeの大部分は，新生児期から小児期にかけて発症するが^1,10,11），稀に成人期に初めてこれらの症状が発現する場合もある^3，13）．これらは臨床病態として興味ある一群をなしているがその臨床像に関する詳細はよく知られていない．われわれは腰仙部脂肪脊髄髄膜瘤によりtethered cord syn—dromeが成人期に発症した症例を経験した．本症例の報告と共に過去の報告例を検討し，成人期に発症したtethered cord syndromeの臨床所見・tetheringの原因疾患・手術成績及び成因の特徴について報告する．

A 47-year-old female was admitted with complaints of spontaneous pain at the medial surface of the right thigh, and worsening of urinary incontinence and weak-ness of the legs. These phenomena had been noticed previously 20 years ago, and again 10 years ago, re-spectively.

There was a scar like skin abnormality of 4 × 6 cm over the lumbosacral region.

Compression on this region induced neuralgic pain around the anus radiating down to both legs. Neurologically, weakness of the legs, sensory impair-ment on the L₂ - S₅ dermatomes and urinary inconti-nence were reconginzed.
