開頭術後Subdural tension pneumocephalusの1例

A Case of Postoperative Subdural Tension Pneumocephalus

I．はじめに

　CT scanの普及した現在，開頭術後にpneumo-cephalusが見られることは特に珍しい現象ではなく，またその多くは何ら特別な治療を要しない．しかし，稀にmass effectや神経症状を呈することがあり，この病態はtension pneumocephalusと呼ばれ^16），その原因の大部分は外傷によるものであるが，手術操作に起因した報告も散見される^17）．最近われわれは，破裂脳動脈瘤に対し，neck clippingを施行すると同時にV-P shuntを行ったところ，その6ヵ月後にsubdural tension　pneumocephalusをきたした1例を経験したので，反省点やその発生メカニズムにつき若干の文献的考察を加え報告する．

In this paper, a case with subdural tension pneumo-cephalus secondary to bifrontal craniotomy and VP shunt for ruptured Acorn aneurysm is reported. In this patient, the mechanisms for entry of air into the subdural space and producing mass effect (Tension pneumocephalus) seem to be one way valve mechanism and negative pressure due to excess of CSF drainage owing to shunting system.

Only 29 reported cases of tension pneumocephalus following surgery were found in an extensive review of the literature.
