血液透析患者の聴神経腫瘍手術

Surgery of Acoustic Neurinoma in a Hemodialyzed Patient

I．はじめに

　近年，血液透析法や患者管理の進歩によって透析患者の数は年々増加し^6），かつ長期生存例も多くなってきている．そのため慢性透析療法中に他の疾患を合併し，外科手術の必要にせまられる患者も増加しているが，現在では全身状態や諸検査成績が一定の条件を満たせば手術を行うというのが一般的な見解である^{10, 12）}．しかし，脳神経外科領域においては，透析患者の手術報告例は少なく，特に脳腫瘍手術の報告はわれわれの渉猟しえた限り1例をみるにすぎず^9），いまだ定見はない．今回われわれは，透析患者に合併した聴神経腫瘍の1手術例を経験したので，透析患者における脳神経外科手術に関して若干の考察を加え報告する．

Successful surgical treatment of acoustic neurinoma in a case of hemodialyzed patient is reported.

A 42-year-old female patient, who had been treated by hemodialysis, was diagnosed as having a C-P angle tumor by CT scan. She was refered to our clinic on June 24, 1983.

Laboratory examinations on admission showed severe anemia and renal dysfunction. Every possible treatment was done in order to improve the labora-tory data preoperatively.

Another big problem in this case of hemodialyzed patient was brain edema and bleeding tendency.
