Ⅰ．はじめに

　頭蓋内脂肪腫は，脳腫瘍の中でも頻度の少ない腫瘍であり，その大半は脳梁部に発生するといわれている^18）．1939年Sosman^13）が頭蓋単純写上の特徴ある所見を報告して以来，診断学的には興味ある対象ではあるが，外科手術の対象としては批判的な報告が占めてきた^{4,5,12,13,14,17）}．

　私どもは，自動症を伴う意識消失発作を有し，頭蓋単純写でも典型的な所見を示した脳梁脂肪腫を亜全剔出し，症状を消失させることに成功した1例を報告する．

The patient was a 13-year-old right handed school boy having a lipoma of the corpus callosum with agenesis of its anterior part. Despite anticonvulsive therapy for one and a half years before admission, his spells of automatism became more frequent and he was admitted to our neurosurgical department.

Diagnosis was made by plain skull x-rays, in which a typical radiolucent area in the midline with calcification in its wall was found. Tomography of the skull in lateral view showed a comma-shaped radiolucent area with calcification in the anterior part of the corpus callosum.
