Ⅰ．はじめに

　最も難かしいといわれている椎骨脳底動脈瘤の直接手術は，その歴史も約30年と比較的新しく，この間諸家により麻酔方法，手術適応およびその拡大法，接近方法，手術手技等，種々論議され，手術成績も向上しつつある．われわれは，これまでこの部の単発性動脈瘤23例を経験しているが，本稿ではこれら症例の外科的治療に関し検討を試み，特に手術の要点につき報告する．

23 cases of vertebrobasilar single aneurysms submitted to surgical operations were studied. 17 were basilar aneurysms and 6 were vertebral aneurysms. 11 cases of multiple aneurysms associated with vertebrobasilar aneurysms are reported in another paper.

Subtemporal approach was adopted to the basilar aneurysms and lateral cerebellar approach following the unilateral suboccipital craniectomy to the vertebral aneurysms.
