Ⅰ．はじめに

　浅側頭動脈（STA）—中大脳動脈皮質枝（cortical MCA）のbypass surgeryはcerebral ischemiaの治療において，今や確固たる位置を占めている．このbypass surgeryは，我々のクリニックでは，1967年の10月以来（Yasargilによる最初の手術^10））今日まで，76の症例に行われ，世界では，その数500以上を数える^2）．そしてこの手術に関する利点がこれまで数多く報告されてきた．手術死亡率は非常に低いと言われ（3％以下）^11），術後合併症が少ないこともまた良く知られていることである．

　最近我々のクリニックにおいて，bypass surgeryの術後合併症として，小脳内血腫（剖検にて確認）を起こし，術後2日以内に死亡した2例を経験したので，ここに報告し，あらためて，この種の合併症に注意を換起したいと思う．というのも，小脳内血腫は，迅速適確な診断，手術処置により治療できるものがあるからである^7）．

Since October 1967 we have performed 76 microsurgical STA-cortical MCA bypass operations.

Recently we have had two cases, who died from intracerebellar hematoma following bypass operation.

Intracerebellar hematoma is reported primarily due to hypertension (50-80%), and to comprise 10%, of all spontaneous intracranial hematomas.

Diagnosis of this lesion is frequently missed but can be made by the typical clinical picture (respiratory irregularity, pinpoint pupils, absence of oculovestibular responses, loss of consciousness), and the CT-Scan.
