Ⅰ．はじめに

　近年脊髄動静脈奇形の外科的療法は選択的脊髄動脈造影の発達とmicrosurgical techniqueの導入により飛躍的に向上し，全摘出が可能なものも数多くみられ，本邦でもその報告が増加しつつある．しかし本邦例をも含めて，"single coiled type"の全摘出の報告が多く，intramedullary nidusをもつ"glomus type"の手術症例の報告は比較的少なく，全摘出は不可能とさえいわれている．

　われわれは最近前根動脈を輸入血管とし，intramedullary nidusをもち，前および後脊髄静脈を輸出血管とした脊髄動静脈奇形，すなわちDiChiroらのいうglomus typeの症例に輸入血管の電気凝固と亜全摘出の二者併用を試み，症状の改善をみたので若干の文献的考察を加え報告する．

A case of spinal cord arteriovenous malformation was reported, in whom serial selective spinal angiogram and pantopaque myelogram showed a successful demonstration of intramedullary nidus.

A 25-year-old male was admitted with paraparesis, impotence, hypesthesia and hypalgesia in his legs in 1974. He was diagnosed to have a spinal cord arteriovenous malformation of so-called "glomus type" with intramedullary nidus by the selective spinal angiogram and pantopaque myelogram.
