脳浮腫に対する副腎皮質ステロイド療法に関する基礎的研究

Basic Studies in Cerebral Edema in Relation to Corticosteroid Therapy

　脳は腫瘍あるいは血腫などによる圧迫，脳出血，外傷，脳循環障害，中毒，炎症などの多くの非生理的状態におかれることにより，一次的あるいは二次的にその体積の増大，膨化をおこす。このような現象を他の組織におけるものにならい，習慣的に脳浮腫と呼びならわしている。この名称はかつてReichardt^35）およびZulch^{52），53）}らにより水分の貯溜が細胞外におこるかあるいは細胞内であるかによつて狭義の脳浮腫と脳腫脹とに区別することが提唱されたこともあつたが，電子顕微鏡的所見などから，成熟脳には光学顕微鏡的な従来の意味での細胞外腔がほとんど存在しないことが多数説となり，さらに同一の脳損傷でも時期的な変化により，細胞外腔に水分が貯溜しているように見えることもあり，根本的に差異はないという説^{43），13）}も見られ，あえて概念的には脳浮腫と脳腫脹とを区別することなく，脳浮腫と呼ぶ傾向があるので，著者もこれに従つている。

1) Effect of corticosteroid in different doses on oil-emboli-induced cerebral edema was studied. Initial dose of hydrocortisone may be around 10 mg/kg to give protective effect on "Blood-Brain Barrier", and smaller doses may not exhibit signifi-cant effect.

2) Different ster oid regimens were tried. Delayed startof steroid administration reduces its effect.

3) Effect of prednisolone on cellular membrane was examined by employing cultured astrocytoma cells. Swelling and vacuole-formation of astro-cytoma cellswere prevented by addition of pre-dnisolone to culture medium.
