Behavior of respiratory neurons during vomiting

はじめに

　嘔吐は胃の内容物を食道を介して逆流させる反射であり，めまいに随伴する症状ばかりでなく，動揺病，妊娠，さらには放射線治療や抗癌剤の副作用など様々な刺激によって誘発されることが知られている。しかし，嘔吐反射を形成する中枢神経機構についてはいまだに十分な解明がなされておらず不明な点が数多く残されている。

　嘔吐反射はからあげに代表されるretching期と，実際に胃内容物を口腔より噴出するexpulsion期の二相によって構成される^34）。これらretchingおよびexpulsion期において，横隔膜，外肋間筋，腹筋などの主要呼吸筋群と上気道筋群は特徴的な活動様式を示すことが知られている^34）。すなわち，吸気筋である横隔膜，外肋間筋は呼気筋である腹筋と同期して律動的に収縮し，胸腔および腹腔内圧を急激に上昇させる。その結果として胃の内容物が食道を介して口腔より逆流されることになる。

Vomiting is a nonrespiratory behavior during which the major respiratory muscles contract in a characteristic pattern that generates the massive changes in intra-thoracic and intra-abdominal pressures that lead to expulsion of gastric contents. During vomiting, the diaphragm and external intercostal muscles co-contract with abdominal expiratory muscles in a series of bursts of activity. On the other hand, internal intercostal muscles contract out of phase with these muscles during retching phase and are inactive during expulsion phase.
