コンサルテーションの現状

Medical Consultation : The Status Quo

　複数の医師の間で患者を紹介し合うことは，日常診療の中で頻繁に行われる重要な医療行為である^1）．このような患者紹介（コンサルテーション）が必ずしも効果的に行われていないことはしばしば指摘はされているものの^2），研究テーマとして扱われることは比較的少ない．特にわが国では，コンサルテーションについての体系立った臨床教育すら行われてこなかった．医療が専門分化し細片化すればするほどコンサルテーションは必要となり，とりわけgeneralistにとっては重要な臨床技量となる．

　本稿では，これまでに蓄積されてきたコンサルテーションに関する知識をまとめ，わが国の現状を考える一助とする．

Consultation has been increasingly recognized as an important clinical activity as specialization of medicine proceeded in the past several decades. It is often said, however, that consultation is not always performed in an effective way to improve patient care. In fact, there have been only a few researches done on consultation and, in particular, almost none in Japan.

This article delineates essential components of effective consultation for both referring and consultant physicians. Subjects dealt with here were as follows.
