播種性黄色腫とその周辺

DISSEMINATED XANTHOMA AND ITS CIRCUMFERENCE

　播種性黄色腫（Xanthoma disseminatum）は，正脂血性皮膚粘膜黄色腫のまれな1型で，四肢屈側や間擦部に好発し，しばしば尿崩症を伴なうものである．発疹は丘疹，あるいは結節性で慢性に経過し，予後良好の疾患であるが，粘膜，特に結膜，角膜，口腔，咽頭，喉頭，気管などにも出現し，時に気道閉塞という重大な合併症を伴ない，気管切開が必要となることがある．発疹は，通常長期にわたつて存在するが，時に自然消褪をみる．

　組織学的には，真皮における組織球，泡沫細胞，多数のTouton型巨細胞および炎症性細胞よりなる肉芽腫性病変で，初期の段階では，組織球の浸潤が主体をなすことから，本症の一次的病変として，肉芽腫形成性の網内系組織球浸潤（一種の細網内皮症）が挙げられ，組織球への脂質蓄積は二次的なものとされている^1）．したがつて，他の組織球浸潤性疾患，特にHistiocytosis X, Ncvoxanthocndothelioma, Histiocytoma cutis, Reticulohistiocytomaとの関連が問題となる．著者らは，本症と診断した1例を報告し，これと近縁の疾患との関連につき考察を加えたい．

Xanthoma disseminatum is sometimes confused to differentiate from Nevoxanthoendothelioma and Hand-Schtiller-Christian disease becaus of the primary changes of these diseases are histiocytic infiltration to reticular tissue, but its typical case is benign chronic normolipemic mucocutaneous Xanthoma and can be differentiate from these two diseases.

A case of Xanthoma disseminatum treated successfully with Cortiosteroid was reported.

This case syggests the possibility that Xanthoma disseminatum is a kind of reticuloendotheliosis.
