はじめに

　皮膚癌の治療に関しては，その治療を行なう医師により異なる。しかし，一般に放射線治療，外科的治療あるいは抗癌剤による治療と大別できよう。勿論しばしば上記3者の併用療法も行なわれる。そのいずれかがより優れているかについては多くの論議があらうが，放射線治療に関しては，上野の限界線治療が知られ，また著者も，しばしば電子線治療について報告している。外科的治療に関しては池田の報告が知られ，抗癌剤に関しては最近Bleomycinが登場している。これらについての詳細は，各識者の文献あるいは報告にゆずるとして，ここでは自験例を中心として，いわゆる基底細胞癌と有棘細胞癌を対象とする皮膚癌の外科的治療について記載する。なお，外科的治療と放射線治療（特に電子線）との併用についても言及し，その経過について記述する。

　最近6年弱において，国立がんセンターに来院した皮膚悪性腫瘍患者は259例に及び，その内訳は有棘細胞癌156例，基底細胞癌41例，パージエット病9例，ボーエン病3例，悪性黒色腫22例，腺癌4例，カポジー肉腫1例，悪性淋巴腫19例である。

Forty-six cases of squamous cell carcinoma and 19 cases of basal cell epithelioma have been treated surgically for recent 6 years.

Squamous cell carcinoma with surrounding normal skin of 2-3 cm width was resected success-fully.

It was rather important that the depth of removed masses including tumors was deep enough.

If necessary, periost or parts of bone should be removed for this purpose.

Basal cell epithelioma with surrounding normal skin of 1 cm width was resected successfully.

There are two kinds of radiation treatment accompanied with surgical treatment of carcinoma of the skin-preoperative and postoperative radiation. It was helpful to lessen the damage from radiation-ulceration and scar contraction-, and to promote the effect of treatment.

It must be stressed that there was no case of recurrence in squamous cell carcinoma and basal cell epithelioma which received postoperative radiation.
