肥満を伴つた良性黒色表皮腫の1例

BENIGN ACANTHOSIS NIGRICANS ACCOMPANIED WITH OBESITY

I．緒言

　Acanthosis nigricans黒色表皮腫（黒色棘細胞増殖症）はJanovsky^1）とPollitzer^2）がそれぞれ乳嘴状増殖，色素沈着，角質増殖を臨床上の特長とし，それが腋窩，項，頸，外陰などに主に好発する疾患と記載し，UnnaがAcanthosis　nigricansと命名したが，本症にはDystrophie　papillaire et pigmentaire Darier, Melano-dermie papillaire, Keratosis nigricans，などの異名があるが，一般にAcanthosis　nigricans （以下ANと略す）が多く用いられている。DarierがAN症例中に内臓癌の合併を指摘して以来，高齢者にしばしば悪性腫瘍を伴うものが観察される一方，比較的若年者で悪性腫瘍を伴わない症例も多いことから，Bogr-ow^3）は本症を悪性腫瘍合併の有無により悪性型maligne AN （MANと略す）。良性型benigne　AN （BANと略す）に分類した。またANの年齢的分布が思春期と中年以降の高齢者にピークを示すことから，若年型，老年型に分類することもあるが，これはBAN, MANとは必らずしも一致しない。Miescher^4）はかかる症候学的差異に関係なく病因論的立場から原発性本態性型と続発性症候性型とに分類し，向井は癌種を伴わないANが真性型で，伴うものは症候型として別個に取扱うべきとした。Curth^5）6）は若年者にも悪性腫瘍を伴うものがあり，成人でもそれを伴わないものもある所から，若年型，成人型という分類よりも，良性型，悪性型に分類する方が適切であるとし，さらに従来良性型あるいは若年型と診断されるものの中に肥満と平行して皮疹の消長がみられるものがあり，これをPseudoacanthosis nig-ricans （PANと略す）と命名，悪性型，良性型に対する第3型に分類した。その後PANは一応その独立性は認められながらも，Heite u．von　der Heydt^7）はANを悪性型と良性ANに分け，後者は狭義のBANと良性群の特殊型としてPAN, Papillomatose confluente et rét-iculée, Pseudoatrophodermia colli, Parakéra-tose bzw．　Atrophie brillanteを属せしめたごとく，PANはなおBANやその周辺の疾患との関連について論議されている。われわれは最近肥満を伴つた若年性のANの1例を経験したので，その症例の大要を記載し，BANとPANとの関連につき若干の考察を加えた。

A 14-year-old girl had had a tendency to besity since 5 years which was accompanied with dark brown pigmentation around the neck. Three years ago, the pigmented lesions were also found in the axillary, genito-femoral, inframammary, intermammary and popliteal regions symmetrically. At her first visit, the patient was obese, with body weight of 77kg. The fat was deposited evenly on the trunk and extremities. In the cervical, axillary, inframammary, intermammary, genito-femoral, cubital and popliteal regions there were dark brown papillary lesions with pronounced lichenification.

Histologic specimen showed marked hyperkeratosis and papillomatosis with elongation of the papillae, slight acanthosis and partial melanization in the basal cell layer.

Laboratory tests revealed diabetes mellitus and hypertension.

Influence of reducement of the body weight on the skin manifestations is to be followed up and to be reported.
