　はじめに

　喉頭血管腫はしばしばみられる疾患であるが，その治療法については定説がなく，大きさ，局在に応じて適宜選択されている。喉頭以外の耳鼻咽喉科領域の血管腫では，大きさが小さければ経過を観察していくのも1つの方法である。半数以上に自然消退がみられるという報告もある^1）。しかし喉頭の場合，気道であることや出血の危険性から摘出術が第1選択となると考えられる。摘出術に際して，気道の確保や出血したときの対応が最も大切である。手術術式としては，喉頭直達鏡下にレーザーを用いて摘出可能な症例もあるが，腫瘍がある程度大きいものや，部位によっては頸部外切開で喉頭腔に達して十分な術野を確保して摘出するのが安全，確実である。

　今回，喉頭披裂部後外側から発生した血管腫に対して，下咽頭側切開術を用いて摘出した症例を経験したので，若干の文献的考察を加えて報告する。

A 53-year-old man was complaining of foreign body sensation of the pharynx.

A mass was found in the lateral wall of the right arytenoid region and localized in the piriformrecess. Laryngoscopic and CT findings suggested a laryngeal hemangioma. We visualized the tumor by a lateral hypopharyngotomy. The tumor was removed with a small amount of bleeding. This tumor was diagnosed as cavernous hemangioma by histological examination. Since a lateral hypo-pharyngotomy provides a wide surgical field with minimal surgical stress, it would be a safe surgical approach for laryngeal lesions such as vascular-rich tumors.
