　はじめに

　鼻・副鼻腔癌が頭蓋底に浸潤した場合，従来の拡大上顎全摘のような外頭蓋底からのアプローチのみでは，癌の一塊切除は不可能である。われわれは1983年より頭蓋底に浸潤した鼻・副鼻腔癌に対して，開頭操作を併用したCombined approachによる前・中頭蓋底手術を12症例に経験している（表1）。

　今回は，前頭蓋底手術を施行した7症例^1,2）のうち，最近の2症例について，手術手技を中心に，再建方法および予後について検討を加えたので報告する。

The combined craniofacial approach was per-formed for en bloc resection of the anterior and middle skull base invasion of carcinoma of the nasal cavity and paranasal sinus.

In 12 patients treated with these procedures by the head and neck surgeons, seven cases of the anterior skull base were described.

In two cases of the anterior skull base tumor (the frontal sinus carcinoma, and the maxillary sinus carcinoma involving the ethmoid sinus), the method of combined craniofacial resection, recon-struction and its results were reported.
