はじめに

　サルコイドーシスは肺門縦隔リンパ節，肺，眼，皮膚，神経，筋，心臓，腎臓，骨，消化器など全身のさまざまな臓器に非乾酪性の類上皮細胞肉芽腫を形成する原因不明の多臓器疾患である。発症時の臨床症状は多彩で，その後の臨床経過も多様である^1）。病変が咽喉頭に生じたものは咽喉頭サルコイドーシスと呼ばれ，特徴的臨床所見に乏しいことから，多くは病理組織学的検査によって診断される。

　今回，急性口蓋扁桃炎，急性喉頭蓋炎として抗菌薬やステロイドによる治療を行ったが改善せず，口蓋扁桃の生検を施行し，最終的に咽喉頭サルコイドーシスと診断した症例を経験したので，文献的考察を加えて報告する。

　Here, we report a rare case of pharyngeal and laryngeal sarcoidosis. A 36-year-old woman was referred to our hospital with complaints of a sore throat and discomfort during swallowing. She was found to have swelling of the middle pharynx and epiglottis in the arytenoid region, which did not improve despite treatment with antimicrobials and steroids. A biopsy was performed under local anesthesia. A histological examination confirmed pharyngeal and laryngeal sarcoidosis. Because there were no systemic complications and the symptoms were minor, she was followed-up with special attention to the upper airway stenosis.
