Abstract 文献概要

　12歳女児が3週間前に両眼の視力低下を自覚し、他医で虹彩炎と視神経乳頭炎と診断された。当科受診時の矯正視力は右0.1，左0.09であった。両眼に前房の細胞遊走，虹彩後癒着，前部硝子体混濁，視神経乳頭の腫脹と発赤があり，周辺に網脈絡膜滲出斑が散在していた。両眼に中心暗点があり，左眼の下方視野に狭窄があった。問診で野良ネコとの接触があり，Bartonella henselaeの血清抗体価が上昇していたため，ネコひっかき病を疑った。抗菌薬とステロイドの全身投与を行い，視力がやや改善し，乳頭腫脹が軽減した。ネコひっかき病の診断基準と治療法はまだ確立されていない現在，参考になる症例である。

A 12-year-old girl presented with blurring of vision in both eyes 3 weeks before. Her corrected visual acuitywas 0.1 right and 0.09 left. Both eyes showed cells in the anterior chamber, posterior synechia, anterior vitreous opacity, swelling of optic disc and multiple chorioretinal exudates in the periphery. Central scotoma was present in both eyes andinferior scotoma in the left eye. She had a history of contact with stray cats. She was diagnosed with cat scratch disease after her serum proved to be positive for antibody to Bartonella henselae. Systemic antifungal agents and corticosteroidsresulted in slightly improved fundus findings and visual acuity. This case may serve as an example regarding the diagnosis and therapeutics of cat scratch disease which are still controversial.
