Engelmann病の1症例

A case of Engelmann's disease

　Engelmann病による視神経症の1例を報告した。症例は24歳男性で，小児期よりEngelmann病の診断を受けていたが，眼の掻痒感と流涙にて眼科を受診したところ，視神経乳頭浮腫が認められた。視野およびCT所見より，視神経管の狭細化による視神経症と診断した。ステロイド療法を施行したところ，視野所見の著明な改善がみられた。Engelmann病の眼所見に関する治療の報告は稀で，手術療法が主であるが，症例によつてはステロイド療法が有効であると考えられた。

A 24-year-old male had been diagnosed, at the age of 11 years, as Engelmann disease, or progressive diaphyseal dyslasia. Bilateral optic disc edema was detected when he recently sought medical advice for ocular itchiness and epiphora. Computerized tomography showed cranial hyperostosis and narrowing of the optic canal. Additional perimetric findings led to the diagnosis of optic neuropathy. Systemic corticosteroid induced improve-ment in the visual field. This case illustrates that systemic corticosteroid may be effective for optic neuropathy in this disease.
