　61歳の男性が右眼の視力低下と異物感を主訴に近医を受診した。右眼の角膜上皮障害に対し角膜保護剤の点眼にて加療されたが，改善しないため当科に紹介された。アレルギー疾患の既往歴および家族歴に特記する点はなかった。初診時，両眼に点状表層角膜症，右眼角膜に浸潤があった。上眼瞼結膜には巨大乳頭や線維性の増殖性変化があった。結膜擦過物には多数の好酸球があり，血清学的検査でヤケヒョウヒダニ，ハウスダスト，カビに対する血清中抗原特異的IgE抗体価が上昇していた。これらの所見から，61歳と高齢であったが春季カタルと確定診断した。

A 61-year-old male developed impaired visual acuity and foreigri-body sensation in his right eye.He was referred to us after failure of cornea-protective eyedrops for corneal epithelial lesions.He was free ofallergic manifestations in the past and family histories. Both eyes showed superficial punctate keratopathy andthe right eye showed infiltrations in the cornea.Giant papillae and fibrous hypertrophic tissue were present.Conjunctival scrapings showed numerous eosinophils. Radioallergosorbent test showed elevated serums levelof specific lgE to mite, house dust and mold. These clinical and laboratory findings led to the diagnosis of vernalkeratoconjunctivitis in spite of his advanced age.
