緒　言

　1933年にSjögren^1）が乾性角結膜炎，耳下腺腫脹に慢性関節リウマチ（以下RA）を合併した症例を報告して以来，40年近い歳月を経ているが，特に最近になつて，Sjögren症候群（以下SS）に対する関心がたかまり，日本では1975年難病の一つに指定されている。

　しかし，SSが，涙腺，唾液腺をtarget organとするsicca syndromeを主体とする独立疾患か，Systemic Lupus Erythematodes （以下S-LE）のchronic benignの型か，RAの合併症にすぎないのか，あるいはRAのvarianttypeであるのかなど，SSの疾患としての位置づけは，いまだに明確にされていない現状である。今回，われわれはSSの病態解明の一助として，眼科受診中の原発性乾性角結膜炎（以下KCS），および当付属病院整形外科のRA外来に受診中であるが，眼科には未受診の患者の2群について，臨床病態の比較検討を行ない，いささかの知見を得たので報告する。

In an attempt to clarify the clinical manife-stations of Sjögren's syndrome, we comparedthe clinical features of 50 cases with kerato-conjunctivitis sicca syndrome and 62 cases with chronic rheumatoid arthritis.

In the group with keratoconjunctivitis sicca, 'dry mouth' was present in 76% and 'rheuma-toid arthritis' in 38% of the cases. Further, 16% of the cases manifested collagen or auto-immune diseases other than rheumatoid arthri-tis. A total of 42% of the group with kerato-conjunctivitis sicca thus belonged to 'definite Sjögren's syndrome' after the diagnostic criteria of Vanselow.
