網膜剥離手術後における瞳孔異常について

Pupillary Changes after Retinal Detachment Operation

緒言

　網膜剥離の復位手術として，ジアテルミー凝固とともに強膜短縮，Scleral bucklingなど眼球にかなりの侵襲を加える術式がひろく採用されているので，網膜剥離そのものによる網膜の機能低下の他に，眼球壁に加えられる諸種の手術侵襲の瞳孔に及ぼす影響の存在が推定される。しかるに今迄，網膜剥離手術後瞳孔異常については，Ad—ler and Scheie^3）及びKronfeld^1）らの報告をみるにすぎない。

　私は諸種の術式により行なわれた網膜剥離復位手術成功例について，瞳孔の大きさ，形，対光反応の検査をしたので，その成績について報告する（両眼手術10例を含む）。

The changes in pupillary size and light reac-tion have been studied in 60 eyes of 50 subjects following successful retinal detachment surgery.

The pupil in the operated eye was significant-ly mydriatic than the opposite eye in 23 ins-tances (46%). The grade of pupillary change was positively correlated with the area of dia-thermy coagulation. The possible pressure upon the eyeglobe by scleral infolding or buckling. on the other hand, was found to exert no in-fluence upon the pupillary change. The disturbed light reaction was always present whenever the ultimate visual acuity was less than 0.06.
